1) Pretty sure the "person who was worshipped" is Liz, which I doubt she has any role in creating the Layers.

Why should Liz be worshipped? What if there is only one Liz, like there is only one Mur, and one player?

The plan, to my understanding, is the structure of the layer system or the layer structure itself. The way layers overlap each other, with one point existing in a layer and that same position existing in another. None can exist without the other; when the plan is gone the layer system would have fallen apart, if there are no layers there is no layer structure or plan for the layers necessary for that layer structure to exist does not.

The Plan cannot exist without the layers, but the layers can exist - and in fact in the story the are doing it right now - without the plan existing anymore.

The plan is still in existence, but damaged extensively and is now beyond recognition. Still exists, just in horrible condition – and (side-note:) the plan is not the only thing on the verge of collapse.
