The portal was obviously inside the bulb, it was also told to be Murtuagh's most complex portal so far.

I believe most of them were in the Core, where the lighthouse was. I doubt Murtaugh went far after finish his 'baby steps' in the winter palace. He went into the construction for the lighthouse portal after finding the wisdom gem in the basement. Despite the Karma Portal he used to get there is....missing.

Also, from a outside point of view, Mateusz probably didn't have a clear idea of what a karma portal was gonna be like before Sub7. At least the modified ending in Sub4 HD is evidence of that; most "white sparks" in old games would correspond to the now omnipresent blue/green "energy lines".

Well as we do see in Sub9, karma portals can be contained, so maybe they were able to remove them? Some parts of Sub2 do look... damaged by what could be a karma portal, same goes for Sub4. So perhaps they were removed at some point... hm.

Well, in Sub8 there seems to be a place of a "removed" Karma Portal where we get the Energy Trail.

Always though the MAP symbol was just the logo of Submachine with MAP being letters of names of developers that gave Mateusz inspiration to create Submachine. A obviously being Albartus which is the last name of Jan Albartus the developer of MOTAS which is one of the inspirations for Submachine.
Of course I've never seen an M present, just an A and a P. I bet the reason for said symbol has already been posted, maybe somewhere on the Revolutions Archives or on some other game discussion site for Submachine 1 when it first came out.
The A could also stand for Amanita, which Matuesz has stated several times is a huge fan of their games.
